Can one insert parts or all of the form in another HTML screen? Like a text box part of the form?

Sorry I don't understand what you mean with a text box? May be check-box or text-area?

Lets say I have an .html screen of images and text whatever... I can make a link to the FPM Mailer and user hyperlinks to the new FPM page of the mailer form.

Is there a way to embed the FPM mailer in the "original" screen of images and text?

You can use inline frame to embed the mail-form in a normal html document:

<iframe src="cgi-bin/fpm.cgi" width="100%" height="500"></iframe>

You can also use SSI to call the program from a page. See the FAQ of your provider.

Inline frame works but the server side include does not seem to work. All my pages are done in shtml and my server does ssi. What could be wrong?
