I just discovered your formmailer and think it is a very nice piece of
software: Easy to install, a comfortable admin-interface, flexible and
simple.

But there are a few things I am curious about:

1) fpm.cgi in your zip-file contains dos-line-endings; at least my apache2
under linux didn't like these and produced an internal server error (without
additional information). many beginners might give up on this problem

2) why do you empty the $conttype-string on line 554? without a content-type
browsers (at least my browser) show the pure html-code instead of the
rendered site; i commented out this line and the install-finished-page
appeared correct finally

3) another tip for beginners: you should mention the need for write access
(for the webserver user) to the cgi-bin directory while installing (and
recommend removing write access after the install has finished)
